LA Case Management
XstremeMD is seeking a bilingual (English/Spanish) Licensed Practical Nurse (LPN) Case Manager to coordinate and monitor medical care for injured or ill individuals. This role involves working closely with employers, healthcare providers, insurance carriers, and employees to ensure appropriate treatment, effective communication, and a safe return to work. Candidates must have a current LPN license, strong organizational and communication skills, and the ability to manage multiple cases in a fast-paced environment. Fluency in both Spanish and English is required to support our diverse workforce.
Key Performance Indicators

Attaining a minimum of 95% on the quality score on all audits based on protocol utilization

Meeting department requirement to listen to four calls and complete self-evaluations of calls each quarter – 2 intake and 2 follow-ups

Except for nights and holidays log 7 billable hours per 8-hour shift

Initial follow-up assessment completed in <24 hr.

Maintain nursing license in assigned states

Earn 15 job-related CEUs annually to improve clinical competence

Attend 75% of monthly staff/training meetings
Qualifications
-
LPN with current, valid state licensure required

Minimum of 2+ years clinical nursing experience required

Minimum of 2+ years Emergency Room/ICU experience preferred

Case Management and Workers compensation-related experience preferred

Excellent interpersonal skills, phone etiquette, time management and organizational skills are essential

Must be proficient in Microsoft Office with programs such as Word and Excel, as well as have experience with Internet searches and a comfort level with other software programs

Must be able to type 50 wpm minimum

Ability to set priorities and work independently is essential

Must be able to pass a background check and drug test, as applicable for the role
Working Conditions

Typically, 8-12 hour per day shift work, with the possibility of evening, night, weekend and holiday hours.

Attend meetings and work with staff to complete projects and work assignments.

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Case Manager will work remotely from home. Professional work environment must be maintained while on duty.
