All Homecaring Minnesota, a Medicare-certified home health agency based in the Twin Cities, is expanding and looking for a passionate and experienced RN Case Manager to serve clients in the North Suburbs/North of 394 area.

We believe excellent patient care goes beyond the chart. As an RN Case Manager with us, you’ll build long-term relationships and truly see the whole picture of your patients’ lives.
Your Role:
The RN Case Manager plans, organizes, and directs home care services for individuals and families in their homes. You will:
- Conduct full physical assessments and reassessments
- Develop, implement, and revise individualized care plans
- Administer treatments and medications as prescribed
- Educate and support patients and their families
- Supervise home health aides and PCAs
- Serve as a mentor to new RNs
- Participate in rotating on-call duties
- Collaborate with physicians, staff, and caregivers
- Ensure regulatory compliance and documentation
Requirements:
- Current RN license in the state of Minnesota
- Valid driver’s license & proof of car insurance
- Ability to pass a DHS criminal background check (we provide)
- 2-Step TB skin test or blood draw (we can provide)
