Licensed Practical Nurse (LPN)
The Licensed Practical Nurse (LPN) is responsible for preparing patients for examinations and delivering individualized patient care by promoting and restoring health through the nursing process. The LPN collaborates with physicians and multidisciplinary care team members to provide physical and emotional support to patients in a Primary Care setting. Responsibilities include assessing patient health concerns, administering nursing care, maintaining accurate medical records, and supporting continuity of care. The LPN will also contribute to the clinic’s efforts to achieve and maintain Patient-Centered Primary Care Home (PCPCH) recognition and support the effective use and optimization of electronic health records (EHR) and other health information technology systems. Additionally, the LPN will assist in implementing care plans, educating patients on health maintenance and disease prevention, and providing guidance and preventative care within the Scope of Practice for a Licensed Practical Nurse.
ESSENTIAL JOB RESPONSIBILITIES

Escorts patients to exam rooms, prepares them for examination and treatment, obtains vital signs, and enters medical history into the electronic medical record (EMR).

Prepares exam rooms and instruments for examinations and testing andmaintainsappropriate medicalsupply inventory.

Provides direction and support to Medical Assistants (MAs)regardingpatient care activities.

Conducts patient assessments and prioritizes care based on the patient’s condition and needs within the LPN Scope of Practice.

Assistsin developing and implementing individualized care plans in collaboration with providers and healthcare team members.

Performs treatments and therapeutic nursing interventions as ordered by the provider inan accurateandtimelymanner.

Provides age-appropriate, culturally competent patient care and education tailored to individual patient and family needs.

Documents patient assessments, interventions, responses to treatment, and progress toward care goals accurately andtimelyin the medical record.

Verifies patient identity prior to providing or documenting care and supports efficient patient flow and care coordination.

Collectslaboratory specimensin accordance withorganizational guidelines.

Adheres to universal safety precautions, including safe medication administration and proper disposal of medical waste.

Initiates emergency resuscitative measuresin accordance withestablished adult and pediatric life support protocols.

Maintains patient confidentiality and compliance with HIPAA requirements.

Provides care in a nonjudgmental and nondiscriminatory manner that respects patient autonomy, dignity, rights, and diversity.

Maintains professional competency through continuing education and professional development activities.

Assistswith desktop triage and other clinical support functions as assigned.

Complywithorganizational policies, procedures, Code of Conduct, Compliance Plan, and all applicable federal, state, and local regulations.

Performs other duties as assigned to support departmental and organizational operations.

Minimum Qualifications
-
Current Licensed Practical Nurse (LPN) license issued by the State of Oregon.

Current CPR certification.

Demonstrated ability to perform nursing skills within the LPN Scope of Practice.

Current valid driver’s license, reliable transportation, and proof of automobile insurance.
-
All vaccinationsupto date, including butare notlimited to Hepatitis, MMR, and Tetanus. COVID-19 vaccination preferred or approved medical/religious exemption on file.

Ability and willingness to work three (3) 12-hour shifts, including some weekends.

Proficient computer skills, including experience with electronic health records (EHR), Microsoft Office Suite, typing, and 10-key preferred.

No suspension, exclusion, or debarment from participation in federal healthcare programs (e.g., Medicare/Medicaid).

Bilingual or translation capabilities preferred.

Ability to travel and drive to in-person meetings or be on-site asrequired.
