Ultimate Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team. This position provides the opportunity to work with residents in a skilled nursing and post-acute care setting, helping them improve mobility, function, and overall quality of life.
Position Responsibilities:

Conduct physical therapy evaluations and develop individualized treatment plans based on resident needs and goals

Provide therapy services focused on mobility, gait training, balance, strengthening, endurance, transfers, and pain management

Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to coordinate resident care

Monitor resident progress and adjust treatment approaches as appropriate

Educate residents and families on therapy goals, exercises, and safety strategies

Maintain accurate and timely documentation in accordance with facility and regulatory requirements

Qualifications:

Active Physical Therapist license required

Strong clinical reasoning and communication skills

Skilled nursing experience preferred but not required
