Overview:
Are you a clinician who thrives on building relationships, solving problems, and helping patients achieve the best possible outcomes? Use your clinical expertise to impact patients and families without the demands of direct patient care, productivity standards, weekend visits, or endless documentation. We are looking for a passionate clinician to join our team as a Care Coordinator, serving as a trusted clinical resource to hospitals, skilled nursing facilities, rehabilitation centers, physicians, and other healthcare partners throughout the community. This is not traditional sales, this is clinical leadership, patient advocacy, and relationship-building at its finest.

Want to expand their influence beyond direct patient care
Must be a graduate of an approved school of nursing or therapy.

Must be licensed in the state where they currently practice.
- Must have two (2) years of demonstrated field experience.
- Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.

Must have basic demonstrated technology skills, including operation of a mobile device.

Must maintain automobile liability insurance as required by law

Must maintain dependable transportation in good working condition

Must be able to safely drive an automobile in all types of weather conditions
