Job Overview
We are seeking a dedicated and highly skilled MDS Coordinator LVN or RN to join our healthcare team. The ideal candidate will possess a comprehensive understanding of the Minimum Data Set (MDS) process, clinical documentation standards, and regulatory requirements associated with long-term care and skilled nursing facilities. As an MDS Coordinator, you will be responsible for ensuring accurate and timely completion of resident assessments, optimizing reimbursement through precise coding, and maintaining compliance with federal and state healthcare regulations. Your expertise in clinical documentation improvement and utilization management will support high-quality patient care and operational excellence.
Duties
- Conduct comprehensive MDS assessments in accordance with federal regulations, including CMS guidelines, ensuring accuracy and completeness.
- Review medical records, clinical documentation, and care plans to verify data integrity and adherence to documentation standards.
- Collaborate with interdisciplinary teams—including nursing, therapy, social services, and medical staff—to gather relevant resident information for assessments.
- Ensure timely submission of MDS assessments to facilitate appropriate reimbursement and quality reporting.
- Utilize EMR/EHR systems such as Epic, Cerner, or Athenahealth to document assessments accurately within electronic health records.
- Maintain up-to-date knowledge of Medicare, Medicaid, ICD-10/ICD-9 coding, CPT coding, DRG classifications, and long-term care regulations.
- Support compliance with HIPAA privacy standards and health regulation policies while managing sensitive medical records.
- Participate in quality assurance activities related to clinical documentation improvement and utilization review processes.
- Assist in discharge planning, case management, and coordination of care for residents requiring specialized services such as hospice or emergency medicine.
Requirements
- Valid LVN or RN licensure with current state certification; additional certifications in case management or health information management are advantageous.
- Proven experience in long-term care settings, skilled nursing facilities, or inpatient hospital environments with familiarity in acute care or ICU experience preferred.
- Strong knowledge of CMS regulations, NCQA standards, Medicare/Medicaid policies, and long-term care regulations.
- Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks; experience with Microsoft Office suite is essential.
- Expertise in medical coding including ICD-10/ICD-9/ICD coding standards; CPT coding knowledge is highly desirable.
- Demonstrated ability to review medical documentation critically and ensure compliance with clinical documentation standards.
- Excellent organizational skills with attention to detail; ability to manage multiple assessments simultaneously under tight deadlines.
- Effective communication skills to collaborate across multidisciplinary teams while maintaining confidentiality per HIPAA guidelines.
