1st Americare is seeking an LPN to work with our client in the Arlington area. Client lives in home setting and will need help with all ADLs including transferring. Family is Cantonese-Speaking.
The schedule for this case will be:
- Wednesdays & Thursdays 9am-3pm
Duties and Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient care activities accurately and timely
Job Type: Full-time
Pay: $35.00 - $45.00 per hour
