About This Role
We are looking for a compassionate, skilled RN or LPN to join our assisted living team. You will provide direct nursing care and wellness oversight to our residents, support their independence and dignity, and serve as a clinical resource for our care team. What You'll Do
Conduct resident wellness assessments and monitor changes in condition
Administer medications and treatments per physician orders and ALF regulations
Develop and maintain individualized service plans in collaboration with the care team
Communicate changes in resident condition to physicians, families, and the care team promptly
Supervise and delegate tasks to medication aides and care staff on your shift
Perform skilled nursing tasks including wound care, treatments, and catheter care
Coordinate with physicians, home health, hospice, and outside providers as needed
Ensure compliance with state ALF regulations and community policies
Document care accurately and timely in the electronic health record
Participate in care planning, quality improvement, and resident satisfaction initiatives
Requirements
Current, unrestricted RN or LPN license in the state of practice
Graduate of an accredited nursing program
Current BLS/CPR certification
Assisted living, long-term care, or geriatric nursing experience preferred
Strong assessment, communication, and organizational skills
Genuine compassion for elderly residents and their families
Ability to work rotating shifts, weekends, and holidays
