Mt. Ogden Health & Rehabilitation is seeking a professional, organized, and people-focused Transitional Care Coordinator to join our high-performing team.
This position plays an important role in helping new residents and their families experience a smooth transition into our facility. The Transitional Care Coordinator will work closely with referral sources, hospital and community partners, facility leadership, nursing, therapy, and other departments to help coordinate admissions and ensure our team is prepared to welcome each new resident.

Key Responsibilities
- Serve as a professional and responsive point of contact for referral sources, hospitals, families, and community partners.
- Assist with communication regarding incoming referrals and potential admissions.
- Help coordinate the admission process from initial referral through resident arrival.
- Communicate upcoming admissions with nursing, therapy, dietary, housekeeping, business office, and other appropriate departments.
- Help ensure resident rooms and the facility are prepared prior to admission.
- Gather and communicate important admission information to the appropriate members of the interdisciplinary team.
- Maintain timely communication with residents, families, referral partners, and internal team members throughout the transition process.
- Assist residents and families upon arrival and help create a welcoming, organized admission experience.
- Participate in a rotating weekend schedule to assist with weekend admissions and referral communication.
- Maintain appropriate confidentiality and follow HIPAA and facility policies when handling resident information.
- Build positive relationships with hospital case managers, discharge planners, referral partners, residents, and families.
- Represent Mt. Ogden Health & Rehabilitation professionally and positively in all interactions, both inside and outside the facility.
- Assist the leadership and admissions teams with additional projects and responsibilities as needed.

Job Type: Full-time
Pay: $40,000.00 - $60,000.00 per year
