POSITION OVERVIEW:
The Home Health Aide (CNA) provides direct personal care and supportive services to patients in their homes under the supervision of a registered nurse or therapist. This role assists patients with activities of daily living, monitors and reports changes in condition, and helps maintain a safe, clean, and comfortable environment. A few position highlights include:
- Provide personal care and assistance with daily living activities, including bathing, grooming, toileting, and mobility.
- Take and record vital signs (temperature, pulse, blood pressure, and respiration) and report any changes in patient condition.
- Support patients with meals, light housekeeping, and maintaining a clean, safe, and healthy home environment.
- Observe, document, and report patient status changes, and follow care center procedures for safety, infection control, and emergency response.
- Establish and maintain an effective and efficient visit route.
THE MUST HAVES:
- High School Diploma or equivalent.
- Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
- CNA experience within the last two (2) years. If it has been longer, you must complete another training and competency program before providing services.
- Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
