Position Summary:
Toria's Support Care Services is seeking a highly motivated and experienced Licensed Practical Nurse (LPN) to serve as a Facility Supervisor and Utilization Review Specialist. This critical role provides clinical oversight, utilization review, and quality-improvement leadership across our facilities. The LPN Facility Supervisor is instrumental in ensuring that all nursing practice, documentation, and staff competencies meet the stringent requirements of the iBudget Waiver’s Residential Nursing (continuous care), Skilled Nursing (intermittent care), and Specialized Medical Home Care (24-hour medical group-home) regulations, Rule 65G‑7 F.A.C., and AHCA Residential Care Standards (Effective July 7, 2025).
This position reports directly to the Director of Nursing / Administrator and supervises Medication Technicians and Direct-Care Staff (task delegation under Rule 64B9‑14 F.A.C.).
Core Responsibilities:
Daily:
- Manage consumer health issues and supervise Medication Technicians, including direct administration of injections as needed.
- Conduct whole-home safety walk-throughs and initiate immediate corrective actions for identified hazards or infection control breaches.
- Verify Medication Administration Record (MAR) accuracy before the first pass; observe medication administration and provide coaching to staff to ensure 65G-7 competency.
- Update iConnect/iBudget health & safety records within 24 hours of any change in resident condition.
- Escalate significant clinical findings promptly to physicians, families/guardians, Waiver Support Coordinators (WSCs), and the Administrator.
Weekly / Bi-Weekly:
- Audit seven-day medication-error logs and controlled-substance counts to ensure accuracy and compliance.
- Lead focused coaching and competency refreshers for Direct-Care staff.
- Validate staffing ratios, licensure, and delegated-task competencies against resident acuity and needs.
- Participate in interdisciplinary "huddles" to review upcoming appointments, Individual Plan (IP) goals, infection-control logs, and emerging risk trends.
- Observe fire and disaster drills, validate documentation, and debrief with staff (fire, elopement, severe weather).
Monthly:
- Reconcile physician orders, pharmacy labels, and MARs; sign monthly MAR verification.
- Complete comprehensive binder audits (MAR, Program, Emergency, Elopement, Menu, Fire-Drill).
- Perform Utilization Review (pre-certification, continued stay) for RN, SK, SMH service authorizations.
- Review Individual Plans (IPs) for progress and recommend goal revisions to optimize resident outcomes.
- Chair interdisciplinary safety meetings and issue quality-indicator reports (falls, medication errors, transfers).
- Submit monthly metrics (falls, med errors, hospital transfers, wound prevalence) to the QA Committee.
- Complete pre-certification or continued-stay reviews for residents receiving Skilled Nursing, Private Duty Nursing, or therapies reimbursed under iBudget.
Quarterly:
- Conduct comprehensive file reviews for APD & AHCA compliance, verifying consents, orders, and diagnostics.
- Reassess each resident, update their Plan of Care, and notify WSC of any changes.
- Submit an environmental safety assessment with an action plan for each licensed setting (65G-2 environmental checklist).
Annually:
- Facilitate 4-hour 65G-7 medication re-certification and skills validation for all medication handlers.
- Lead annual review of Nursing, Infection-Control, Emergency, and QA policies, updating to reflect new APD or CMS guidance.
- Perform aggregate incident-trend analysis and draft a comprehensive Quality-Improvement Plan.
- Complete performance evaluations for Medication Technicians and contribute to staff recognition initiatives.
As Needed / PRN:
- Provide direct skilled nursing care during acute changes in resident condition or introduction of new therapies.
- Coordinate specialist referrals; train staff on equipment or regimen changes.
- Prepare for APD/AHCA surveys; draft and monitor Plans of Correction to address deficiencies.
- Liaise with families, physicians, hospitals, pharmacies, and vendors for continuity and cost-effective care.
- Develop corrective-action plans for staff or systemic deficits to enhance quality of care.
- Lead document preparation and onsite support for AHCA/APD surveyors; ensure plans of correction are implemented and re-audited.
- Participate in emergency response (infection outbreaks), new facility openings, or policy roll-outs at the direction of the Administrator.
Additional Duties:
- Support Plan Governance: Quarterly review of Support Plans & Plans of Care to ensure targeted health needs are addressed.
- Consumer Liaison: Serve as a primary point-of-contact for families, caregivers, WSCs, social workers, and outside agencies.
- Staff Development: Conduct regular meetings, coaching sessions, and recommend recognition programs to foster a culture of safety and excellence.
- Documentation Integrity: Ensure clinical records accurately reflect the level-of-service provided; query clinicians for clarification when needed.
- Perform other duties as assigned to maintain compliance and resident well-being.
Minimum Qualifications:
- Current Florida LPN license (unrestricted); IV certification preferred.
- Two (2) years of experience in I/DD residential or long-term care nursing; utilization-review background highly desirable.
- Proficient in iConnect EHR, Microsoft Office Suite, and state reporting portals (e.g., APD Aspen system).
- Working knowledge of iBudget Waiver service definitions, Rule 65G-7 medication standards, and AHCA licensure criteria.
