About the Role:
As a Registered Nurse at Claremont Nursing & Rehabilitation Center, you will have the opportunity to work in a dynamic and supportive environment that values collaboration, teamwork, and patient-centered care. Your responsibilities will include:
Assessing patients' physical, emotional, and social needs, and developing individualized care plans to address these needs
Providing direct patient care, including administering medications, performing treatments, and performing vital signs
Collaborating with other members of the interdisciplinary care team to ensure comprehensive care
Participating in patient education and discharge planning to ensure a smooth transition to home care
Maintaining accurate and up-to-date patient records and reports
