Welsh Mountain Home is seeking a Resident Services Coordinator.

Duties and Responsibilities:
*Manages the referral and admission process for all Welsh Mountain admissions. Develops relationships and maintains regular contact with referral sources to obtain a regular flow of admissions. Demonstrates understanding of the mission and private pay vs subsidized residents.
*Obtain and maintain PCHA license in order to provide back-up services in the absence of Executive Director
*Communicates with residents, family members and staff regarding referrals and admissions. Has the ability to be flexible and to adapt to working in a fast-paced environment.
*Participate in on-call rotation and provide coverage for PCA’s and Med Tech’s as needed
*Manage and oversee implementation of activities calendar and collaborate with PCA’s for activity ideas.
*Responsible for providing transportation and supervision on resident outings
*Coordinate with Director of Wellness to assure all activities are implemented by PCA’s
*Collaborate with Director of Wellness and PCA’s to assist in resident conflict resolution
*Coordinate and manage resident insurance issues and open enrollment
*Coordinate resident services such as Rep Payee, Voter Services and Resident Council meetings
*Be a resource for families and residents for all necessary services
Qualifications:
* A minimum of 60 College Credits or LPN, Bachelor’s degree in Social Work, psychology or health services preferred.
*Must be able to obtain PCA & Med-tech certifications within 90 days of hire
*Must be able to obtain PCHA license within the first two years of employment
*Experience in long term care setting, particularly in admissions or as a clinical liaison, within the past three years required.
*Demonstrates ability to balance and manage challenges of a fast-paced environment.
*Demonstrates a pleasant personality with flexibility and adaptability for frequent interruptions and schedule variations.
*Able to work independently, well-organized, and can multi-task.
*Proficient in use of computer and related technology required for the role.
*Good verbal, non-verbal and written communication skills required.
