Job Overview
We are seeking a dedicated and compassionate Long Term Care Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The ideal candidate will possess a strong background in patient care, with expertise in managing the unique needs of geriatric and disabled populations. As an LPN in this role, you will be responsible for delivering comprehensive nursing services, ensuring patient comfort, safety, and well-being while adhering to all regulatory standards. Your proactive approach and attention to detail will contribute significantly to creating a supportive environment for residents requiring long-term care.
Responsibilities
- Perform detailed patient assessments, monitor vital signs, and document clinical observations accurately within electronic health record (EHR) systems.
- Administer medications, including injections and IV infusions, following prescribed protocols while ensuring compliance with HIPAA and other confidentiality policies.
- Assist residents with activities of daily living such as bathing, dressing, mobility assistance, and catheter care to promote independence and dignity.
- Manage wound care, including dressing changes and infection control measures, particularly for residents with complex needs like pressure ulcers or surgical wounds.
- Collaborate with interdisciplinary teams to develop individualized care plans that address residents' medical, psychological, and social needs.
- Provide specialized dementia and Alzheimer's care, implementing memory care strategies to enhance quality of life for residents with cognitive impairments.
- Support emergency response procedures by utilizing first aid skills and critical care experience when necessary, including airway management and acute pain management.
- Ensure compliance with long-term care regulations and standards while maintaining accurate medical documentation and case management records.
Experience
- Proven experience working as an LPN in long-term care facilities such as skilled nursing facilities or assisted living communities.
- Demonstrated proficiency in EMR/EHR systems like Cerner or Athenahealth for documentation and patient record management.
- Prior experience with geriatrics, dementia care, hospice services, or memory care is highly desirable.
- Familiarity with advanced nursing procedures including IV insertion, catheterization, tube feeding, and sterile techniques.
- Knowledge of ICD coding (ICD-9/ICD-10) for accurate documentation of diagnoses; experience with CPT coding is a plus.
- Background working with individuals with disabilities or developmental delays enhances your ability to deliver personalized resident-centered care.
- Experience in acute or chronic pain management within a hospital or outpatient setting is beneficial for managing complex cases effectively.
