Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor patient health by checking vital signs, observing changes in condition, and reporting findings accurately to healthcare teams
- Support medication management by administering prescribed medications following HIPAA guidelines and care plans
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment for clients
- Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being
- Follow established care plans and document all services provided in compliance with healthcare regulations
Qualifications
- Valid certification as a Home Health Aide (HHA) or equivalent credential from an accredited program
- Experience working in home & community care settings, assisted living facilities, nursing homes, or group homes is highly preferred
- Knowledge of patient observation techniques, dementia care, Alzheimer's care, memory care, and hospice care practices
- Ability to assist with complex tasks such as medication administration, vital signs monitoring, and working with individuals with developmental disabilities or autism
- Strong understanding of HIPAA regulations to ensure client confidentiality at all times
- CPR and First Aid certification required; additional certifications such as Heavy Lifting or Hoyer lift training are advantageous
- Demonstrated compassion for seniors and individuals with disabilities; excellent communication skills; reliable transportation is a plus
