Job Summary
We are seeking a highly skilled and compassionate Licensed Vocational Nurse (LVN) to serve as a Wellness Director in our healthcare facility. The ideal candidate will possess extensive clinical experience, including critical care, long-term care, and acute care settings, with proficiency in EMR/EHR systems such as PCC. This leadership role involves overseeing patient care, coordinating nursing staff, and ensuring compliance with healthcare standards. The Wellness Director will play a vital role in delivering exceptional patient care across diverse medical disciplines including geriatrics, emergency medicine, and specialty areas such as dementia and Alzheimer's care.
Responsibilities
- Provide comprehensive patient assessments, vital signs monitoring, and physical examinations to inform clinical decisions.
- Administer medications and adherence to safety protocols.
- Document all patient interactions accurately utilizing EMR/EHR systems like PCC while maintaining HIPAA compliance.
- Lead and supervise nursing staff during shifts, delegating tasks effectively to ensure high-quality care delivery.
- Conduct diagnostic evaluations such as laboratory specimen collection/processing and medical imaging procedures when necessary.
- Ensure infection control practices are strictly followed to maintain a sterile environment.
- Educate patients and families on health coaching, medication management, nutrition, and disease processes such as dementia or chronic pain management.
Requirements
- Valid LVN license with current state licensure; additional certifications such as Basic Life Support (BLS) required.
- Extensive clinical experience in diverse settings including ICU, Emergency Room, Medical-Surgical units, Geriatrics, Pediatrics (including Neonatal Ventilator), Home & Community Care, Dialysis centers, and Teaching Hospitals.
- Proficiency in EMR/EHR systems such as PCC familiarity with ICD-9/10 coding.
- Strong knowledge of anatomy, physiology, medical terminology, pharmacology related to medication administration and injections.
- Ability to work effectively with diverse populations including elderly patients with dementia or Alzheimer's disease; individuals with disabilities or developmental delays.
- Demonstrated leadership skills in supervising nursing teams during shifts while ensuring compliance with NCQA standards and infection control protocols.
- Ability to perform under pressure in fast-paced environments such as trauma centers or urgent care settings; experience in acute pain management is advantageous. Pay: $66,000.00 - $70,000.00 per year
