General Responsibilities
The Associate Director, Health Services will be responsible for providing supervision to the Care Transitions, Medication Adherence, Social Determinants of Health, Palliative Care and the Health Home teams.
Primary Responsibilities
· Provide direct supervision and leadership to Care Transitions Specialists (CTSs), including:
- Assist with recruitment, onboarding, and training of new staff.
- Identify staff development needs and develop and deliver clinical trainings, refreshers, and ongoing coaching.
- Provide consultation, technical assistance, and guidance to ensure high-quality services and adherence to program standards.
- Support staff in achieving monthly goals, performance targets, and key metrics established by the Senior Director.
- Provide direct Care Transitions services and coverage as needed.
· Support program evaluation, quality improvement, and performance monitoring efforts, using data and analysis to identify trends, opportunities, and areas for improvement.
- Prepare weekly reports for the Care Transitions Program.
· Serve as a program liaison, trainer, and operational support for the Medication Adherence Program, which focuses on improving medication adherence among high-risk, non-adherent members, supporting 90-day medication conversions, and addressing barriers to adherence.
- Support daily program operations, including client prioritization, assignment of staff tasks, monitoring of program performance, and preparation of weekly reports for partner organizations.
- Prepare presentations and lead weekly meetings with partner organizations to provide program updates, review performance, address challenges, and support achievement of program goals.
· Identify and support process improvement, workflow automation, and operational efficiency initiatives across Health Services programs. Collaborate with program leadership to streamline workflows, reduce manual processes, improve data accuracy and reporting, and enhance overall program performance.
· Work closely with the Palliative Care Supportive Services team providing program oversight and guidance to staff to ensure achievement of monthly goals and delivery of high-quality services.
· Oversee Health Home operations and support ongoing program growth, quality, and performance.
· Promote collaboration and integration, as appropriate, across the Health Home, Care Transitions, Medication Adherence, Palliative Care, and other Health Services programs.
· Support the Senior Director of Health Services in the ongoing management, delivery, evaluation, and growth of existing Health Services programs, as well as the development and implementation of new programs and initiatives.
· Strengthen relationships with health plans, hospitals, and community partners, lead partner meetings, and support program growth and business development.
· Perform other duties as requested or assigned.
Qualifications
The Associate Director, Health Services will have the following qualifications:
· International Medical Graduate (IMG), Master's degree in a health-related field, or an equivalent combination of education and relevant professional experience
· Ability to read, write, speak, understand English and Spanish
· At least three years of managerial and leadership experience
· Proven experience in training staff and transferring knowledge and skills to the team
· Desire and ability to work collaboratively and thoughtfully as part of a committed, integrated team
· Excellent interpersonal skills, including the ability to communicate effectively and concisely (both in writing and orally)
· Strong attention to detail with the ability to solve problems, prioritize work, work independently, and excel in a high performance culture
· Ambitious, innovative and creative
· Professional maturity, good judgment and ability to motivate others in a similar way

● Excellent computer skills; familiarity with EHRs and the ability to learn program-specific software. Proficient in Microsoft Office and Google Workspace applications.
● Experience with process improvement, workflow automation, or operational efficiency initiatives. Familiarity with programming languages (e.g., Python), automation tools, AI applications, or data analysis techniques to streamline workflows and improve program operations is highly desirable.
· Ability to travel to clients' homes using mass transit and to negotiate any steps and barriers in clients' residences
· Hospital discharge planning or other health care experience preferred
· An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values have as they relate to serving clients
