Join our dedicated healthcare team as a Home Health Aide working a 4-hour shift, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance in a home or community setting, supporting individuals with diverse needs including seniors, those with disabilities, or individuals receiving hospice or dementia care. Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and document vital signs including blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems
- Support medication administration following proper protocols and maintaining accurate medical documentation in compliance with HIPAA regulations
- Provide specialized care for patients with dementia or Alzheimer’s disease, including memory care strategies and behavioral support techniques
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment in accordance with care plans
- Observe patient conditions closely, report changes promptly to healthcare providers, and update medical records accurately
- Assist with medical procedures such as catheter care or hospice-related activities while maintaining dignity and respect for the patient
Qualifications
- Prior experience in home health care, assisted living facilities, nursing homes or long-term care environments is highly preferred
- Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes
- Certification in CPR is required; additional certifications such as medication administration or hospice care are a plus
- Experience working with individuals with developmental disabilities or dementia enhances your ability to provide compassionate support
- Ability to work independently during short shifts while adhering to all safety protocols and privacy standards (HIPAA)
- Strong communication skills to collaborate effectively with clients, families, and healthcare teams
- Valid driver’s license and reliable transportation are often necessary for community-based assignments
