Duties
- Observe and monitor patients’ conditions regularly, documenting changes accurately in electronic health record (EHR) systems while maintaining strict HIPAA compliance.
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary.
- Administer medications and treatments as prescribed, ensuring proper documentation and adherence to home healthcare regulations.
- Perform vital signs assessments such as blood pressure, pulse, respiration, and temperature to monitor health status.
- Support patients with specialized care needs including Alzheimer’s disease, dementia, hospice, memory care, and end-of-life care.
- Provide assistance with catheter care, wound care, and other medical procedures under supervision.
- Help with meal preparation, feeding assistance, light housekeeping, laundry, and maintaining a clean environment for residents’ safety and comfort.
- Work closely with interdisciplinary teams to coordinate patient care plans while respecting patient rights under HIPAA guidelines.
- Use EMR (Electronic Medical Records) systems efficiently for documentation of patient interactions and medical records.
- Engage in ongoing training on medical terminology, basic life support (BLS), CPR certification, first aid procedures, and home & community care best practices.
Qualifications
- Valid CNA certification or equivalent licensure required; prior assisted living or long-term care experience preferred.
- Proven experience working with geriatric patients or individuals with developmental disabilities is highly desirable.
- Familiarity with Alzheimer’s care, dementia care, hospice care, and memory support techniques.
- Knowledge of medical terminology and proficiency in taking vital signs; experience with catheter care is a plus.
- Ability to lift heavy objects safely and perform physical tasks such as transferring patients using Hoyer lifts or similar equipment.
- Strong communication skills to effectively interact with patients, families, and healthcare teams while maintaining confidentiality per HIPAA standards.
- Experience working within home healthcare regulations and understanding of EHR systems for accurate documentation.
- Compassionate caregiving attitude combined with excellent organizational skills for managing multiple responsibilities efficiently.
