We are seeking an experienced Primary Care Nurse Practitioner to be a foundational member of our brand-new clinic opening in Merced, CA. This is a unique opportunity to help build a patient-centered practice and make a lasting impact on the health and well-being of our community. The candidate will work under the direct supervision of our MD.
The ideal candidate will have a proven track record of providing high-quality primary care in a clinic setting to a broad spectrum of patients from all walks of life. This role involves managing a wide range of health needs, from preventive care to acute and chronic conditions, in a collaborative environment.
Key Responsibilities:
● Conduct comprehensive patient assessments, including history taking, physical exams,
and diagnostic testing.
● Develop and implement individualized treatment plans for patients across all age groups
and demographics.
● Manage acute illnesses, chronic diseases, and preventive health services.
● Collaborate with physicians, nurses, and support staff to ensure seamless patient care.
● Educate patients and families on health management, disease prevention, and wellness
strategies.
● Maintain accurate and timely electronic health records (EHR).
● Participate in quality improvement initiatives and clinic operations as needed.
Qualifications and Requirements:
● Master's or Doctoral degree in Nursing from an accredited program.
● Current Nurse Practitioner certification (FNP or equivalent) and active California RN and
NP license.
● Minimum of 2-3 years of experience as a Nurse Practitioner in a primary care (PCP)
setting.
● Demonstrated ability to serve a diverse patient population, including underserved
communities, with empathy and cultural competence.
● Strong clinical skills in handling a variety of health needs, including family medicine,
pediatrics, women's health, and geriatrics.
● Excellent communication, interpersonal, and organizational skills.
● Proficiency with EHR systems and basic computer applications.
● BLS certification required; ACLS preferred.
Preferred Skills:
● Bilingual abilities (Spanish/English) are a plus, given our diverse patient base.
● Experience in community health settings.
