Responsibilities
- Assist residents with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods.
- Monitor residents’ health by taking vital signs, observing patient conditions, and reporting any changes promptly to healthcare teams.
- Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient confidentiality.
- Prepare nutritious meals and assist with meal consumption, ensuring dietary needs are met in accordance with individual care plans.
- Support residents during recreational activities, outings, and community engagement to promote social interaction and mental stimulation.
- Maintain a clean and safe environment through regular cleaning duties and adherence to health standards in line with home health and hospice care practices.
- Document care activities accurately in resident records, including patient monitoring notes, medication administration logs, and incident reports.

Skills
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments.
- Strong knowledge of dementia care, Alzheimer’s care, memory care, and neurodevelopmental disorders such as autism.
- Skilled in patient observation, vital signs measurement, medication administration, CPR/First Aid certification, and emergency response procedures.
- Ability to assist with ADLs using appropriate manual handling techniques like Hoyer lifts and patient transfer methods.
- Excellent communication skills for engaging effectively with residents, families, and healthcare teams.
- Experience supporting individuals with developmental disabilities or intellectual disabilities with compassionate caregiving approaches.
- Familiarity with HIPAA regulations to ensure confidentiality and privacy of resident information.
