We are currently seeking a Physical Therapist (PT).

Responsible for the assessment of referred patients requiring rehabilitation services.

Provide direct patient care to assess medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.

Establish and administer treatment programs with specific goals determined according to each patient’s capacity and tolerance under the direction of the Physician.

Set realistic and achievable goals for patients.

Document and record patients’ conditions and educate patients and families in appropriate physical therapy methods.

Complete evaluations, weekly progress notes, recertification, and discharge documentation.

Collaborate with all disciplines to provide the best patient-centered treatment and care.

Perform all other duties assigned by the Director of Rehabilitation.

Ensure compliance with confidentiality, HIPAA, and Federal Residents’ Rights.

Graduation from an accredited Physical Therapy college or university.

Valid professional license and registration for the state of practice.

Experience with long-term care and/or geriatric care is preferred but not required.

Experience with Case Mix Index and PPS scheduling is preferred but not required.
