POSITION SUMMARY:
We are looking for an experienced case manager to join our team at Northstar Wellness Center. As a case manager, your role will involve providing support and guidance to inmates at Fresno County jail. Your main responsibilities will include helping inmates access resources and services to support their rehabilitation and successful reintegration into society after release. The program is specifically focused on inmates aged 18-30 who have been or are currently in Fresno County jail, having gang involvement, and are at medium to high risk of reoffending. The goal of this program is to help participants prepare for life after jail with the assistance of case managers, enabling them to lead productive lives and avoid returning to jail. By fostering an environment of trust and encouragement, you will help cultivate resilience, independence, and a sense of agency in each participant. Additionally, you will monitor progress regularly and adjust plans as necessary to ensure that each young person remains on a path toward success. Your efforts will contribute not only to the individual growth of the participants but also to the broader goal of creating a positive future for the community. Through your dedicated work, you will make a meaningful impact in the lives of at-risk youth, guiding them toward a brighter, more hopeful tomorrow.

RESPONSIBILITIES:
· Conduct participant assessment and reassessment to identify criminogenic needs comma barriers comma and strengths
· provide counseling and guidance to inmates regularly
· connect inmates with resources and services, including substance abuse treatment, mental health services, education, and job training programs
· monitor inmate progress and adjust care plans as needed
· maintain accurate and detailed records of inmate interactions and progress
· participate in multidisciplinary teams to address immediate needs and develop strategies for successful reentry
· Develop individualized case plans
· monitor participant compliance with probation, court, treatment, and program requirements
· Track participant outcomes and maintain statistical reports/data and program requirements
SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.

LANGUAGE SKILLS:
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups, and/or boards of directors.
REASONING ABILITY:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exits. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability and skill to deliver psychoeducation material and facilitate groups.
EDUCATION and/or EXPERIENCE:
Bachelor’s degree in Social Work, Psychology, Counseling, or a related field. A Master’s degree is preferred.
Minimum of 2 years of experience in case management, knowledge of criminogenic risk and needs factors impacting justice-involved populations. An ability to work effectively with justice-involved, gang-affiliated, and high-risk populations. Ability to maintain accurate participant records, statistical data using Microsoft Excel and other electronic systems.
CERTIFICATES, LICENSES, REGISTRATIONS:
Must have and maintain a valid California Driver's License, maintain automobile insurance coverage and have access to an automobile.

While performing the duties of this job, the employee is regularly required to talk or hear. WORK ENVIRONMENT:
The work environment is within the community and in a office setting with moderate noise, i.e., computers, printers, and light traffic; additionally will be subject normal child/family noise as well as outside traffic situations.
