The Treatment Coordinator serves as the operational hub of the outpatient program and the patient’s primary point of contact from initial inquiry through discharge. This role is responsible for managing patient access and intake, verifying insurance eligibility, maintaining accurate caseload and roster data, supporting referral development, and contributing to program marketing efforts. As a high-accountability position, the Treatment Coordinator plays a critical role in ensuring efficient conversion of referrals to active care, supporting accurate staffing projections, and sustaining a consistent and growing referral pipeline.
Key ResponsibilitiesPatient Access and Intake
- Serve as the primary point of contact for incoming calls and new patient inquiries, coordinating and scheduling intakes as referral volume increases.
- Manage the intake process from referral through the first appointment, including chart creation and scheduling diagnostic evaluations.
- Ensure all patient charts are complete, accurate, and intake-ready prior to the initial visit.
- Support the Nurse Practitioner and clinical team with scheduling follow-ups and maintaining efficient provider calendars.
Eligibility and Caseload Management
- Verify insurance eligibility at intake and maintain up-to-date eligibility for all active patients.
- Monitor caseload trends and provider capacity, proactively advising leadership on staffing needs.
- Maintain an accurate active roster and ensure timely discharge processing for inactive patients.
Referral Development and Care Coordination
- Build and maintain strong relationships with referral sources, including hospitals, crisis centers, and community partners.
- Identify and develop new referral channels to expand program access.
- Maintain updated resource directories of local providers, specialists, and community services to support care coordination and patient needs.
Marketing and Communications
- Manage the program’s social media, blog, and newsletter to support outreach and community engagement.
- Coordinate staff introductions, program highlights, and group announcements in collaboration with leadership.
- Ensure all communications align with program standards and maintain patient confidentiality.
Leadership and Program Support
- Support leadership with audits, reporting, and program initiatives as needed.
- Perform additional duties as assigned to support overall program operations.
