Job Overview
The primary purpose of your position is to assist in the planning, developing, organizing, implementing, and evaluating our SOCIAL SERVICE PROGRAMS serving Adults with Intellectual Disabilities in accordance with current federal, state, and local requirements, as well as our established policies and procedures, to assure that the medically related emotional and social needs of our residents are met/maintained on an individual basis.
Duties
- Plan, develop, organize, implement, evaluate and direct the social service program of this facility in conjunction with the consulting social worker and Resident Services Director.
- Ensure that the consulting Social Worker signs off on all social histories, social work assessments, and other related documentation as necessary.
- Interview/counsel residents and families as necessary.
- Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required.
- Coordinate all intake, referral, transfer and discharge of facility residents.
- Maintain accurate records through detailed writing skills on client files, reports, and case notes while adhering to confidentiality standards.
- Familiarity with Medicaid, Medicare, Social Security and other government-funded social services systems.
- Demonstrated ability to work effectively with culturally diverse populations; bilingual abilities are highly valued.
- Certifications such as CPR and First Aid are preferred; relevant supervising or mentoring experience is a plus.
- Excellent writing skills for documentation purposes; ability to manage multiple cases efficiently while maintaining attention to detail.
Pay: $41,000.00 - $44,000.00 per year
