Schedule: Full-Time | Monday-Friday (8:00 AM - 5:00 PM) + On-Call and Weekend Rotation
Compensation: $75,000-$100,000 annually DOE
Bonus: $4,000 First-Year Bonus

What You'll Be DoingDeliver Skilled Nursing Care
- Complete skilled nursing visits, assessments, and wound care
- Provide medication management, patient education, and care coordination
- Update care plans and support smooth patient transitions
- Document accurately while maintaining regulatory compliance
- Help patients recover, heal, and maintain independence at home

What You BringRequired
- Active Minnesota RN license
- Reliable transportation and valid driver's license
- Strong assessment, communication, and critical-thinking skills
- Ability to work independently and confidently
Preferred
- Home health, hospice, TCU, or post-acute care experience
- Passion for patient-centered, relationship-based care
