Job Summary:
The Long-Term Acute Care (LTAC) Registered Nurse (RN) provides comprehensive, specialized nursing care to patients with complex medical conditions requiring extended hospitalization. The LTAC RN is responsible for delivering high-quality patient care, collaborating with interdisciplinary teams, and managing critical care interventions in a fast-paced environment.
Key Responsibilities:
- Patient Assessment & Care:
- Conduct thorough patient assessments to identify medical needs.
- Develop, implement, and regularly update individualized care plans.
- Monitor vital signs, respond to changes in patient condition, and intervene as needed.
- Critical Care Management:
- Manage ventilators, tracheostomies, and central lines.
- Administer IV medications, tube feedings, and complex wound care.
- Provide post-surgical care and manage multiple comorbidities.
- Collaboration & Communication:
- Work closely with physicians, therapists, case managers, and other healthcare professionals.
- Communicate effectively with patients and their families regarding care plans and progress.
- Documentation & Compliance:
- Maintain accurate and timely documentation in electronic medical records (EMR).
- Ensure compliance with hospital policies, procedures, and regulatory standards.
- Patient Advocacy & Education:
- Advocate for patient needs to ensure the highest level of care.
- Educate patients and families on disease processes, medications, and discharge plans.
Qualifications:
- Education:
- Associate's or Bachelor’s Degree in Nursing (ADN or BSN).
- Licensure:
- Active and unencumbered RN license
- Certifications:
- BLS (Basic Life Support) certification required.
- ACLS (Advanced Cardiac Life Support) certification preferred.
- Experience:
- Minimum of 1 year of acute care, critical care, or LTAC nursing experience preferred.
