JOB SUMMARY
The Community Liaison is responsible for building and maintaining referral relationships, acquiring new clients, and ensuring overall satisfaction with the organization’s services. This is a field-based, relationship-driven sales role focused on generating a consistent flow of qualified client referrals through proactive outreach and engagement with healthcare and senior care partners. The Community Liaison serves as the primary face of the organization in the local market, building trust with referral sources and converting relationships into long-term clients.
ESSENTIAL DUTIES & RESPONSIBILITIES
Referral Development & Business Development
- Build and maintain relationships with referral sources across the community — including skilled nursing facilities, assisted living communities, home health agencies, hospitals, physician offices, rehabilitation centers, and geriatric care managers — to generate new clients and build brand awareness
- Execute a structured referral marketing process including building and maintaining priority lists, advancing relationships through regular outreach, and tracking activity accurately
- Represent the organization at networking events, community groups, professional committees, and other relevant settings
- Identify new opportunities to expand the referral network and strengthen the organization’s market presence
- Maintain awareness of the competitive landscape and communicate market intelligence to leadership
Client Acquisition & Conversion
- Respond to prospective client enquiries received in-person, by phone, or via web, ensuring each enquiry is handled promptly and professionally
- Learn and conduct client assessments with prospective clients, addressing questions and concerns, building relationships with families, and converting enquiries into long-term clients
- Provide active relationship management for existing referral cases, including home visits, referral partner meetings, and family communications as needed
Relationship Management & Internal Collaboration
- Communicate effectively within the team to ensure care plans meet client needs, referral source satisfaction is maintained, and relevant information is shared in a timely manner
- Support the billing team by helping to ensure client accounts are current — particularly for referrals received from legal and financial advisors — and communicating with families or financial managers as needed
- Contribute to the implementation of new programs and regional or national initiatives as directed
- Perform additional duties as assigned by the supervisor
QUALIFICATIONS
Education
- Preferred: Bachelor's degree in healthcare, marketing, business, or a related field preferred
- Equivalent experience considered
Experience
- Minimum 2–3 years of sales or business development experience, preferably in home care, healthcare, hospice, or a related field
- Proven track record in referral development and relationship building within a field-based sales role
- Preferred: Experience navigating healthcare facilities and engaging effectively with decision-makers and gatekeepers
Skills & Competencies
- Excellent interpersonal, communication, and presentation skills
- Strong sales acumen with the ability to meet and exceed targets consistently
- High level of self-direction and time management — comfortable working independently in a field-based role
- Organized and detail-oriented with the ability to track activity accurately and document in a timely manner
- Computer proficiency and ability to document sales activity accurately in relevant systems
- Emotional intelligence and professional integrity in all interactions
- Current driver's license and proof of insurance
PHYSICAL REQUIREMENTS
- Ability to travel 50–75% of the time across the local market area
- Ability to lift and carry up to 10–15 pounds
- Ability to sit, stand, and walk for prolonged periods throughout the workday
- Ability to use standard office equipment including personal computer, laptop, telephone, and photocopier

Pay: $70,000.00 - $90,000.00 per year
