Job Overview
We are seeking a compassionate and dedicated East Side Hospice Social Worker to join our dynamic healthcare team. Responsibilities
- Conduct patient assessments to determine psychosocial needs, including mental health status, family dynamics, and community resource requirements
- Provide individual, family, and group counseling sessions utilizing clinical techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral health interventions
- Coordinate with healthcare providers and community resources to facilitate discharge planning, home care arrangements, and access to support services
- Document all patient interactions accurately in electronic medical records (EMR systems), ensuring compliance with HIPAA regulations and medical documentation standards
- Offer crisis intervention and emotional support to patients experiencing acute distress or grief, ensuring safety and comfort
- Educate patients and families about hospice care options, medication management, and advance directives in accordance with clinical ethical guidelines
- Collaborate with interdisciplinary team members—including nurses, physicians, chaplains, and therapists—to develop comprehensive care plans tailored to individual needs
Requirements
- Bachelor’s or Master’s degree in social work (BSW or MSW) from an accredited program; licensure or certification as required by state law
- Proven experience working with geriatric patients, end-of-life care, or hospice environments; inpatient or home & community care experience preferred
- Strong knowledge of hospice & palliative medicine practices, medical terminology, ICD coding (ICD-10), Medicare regulations, and medical records management
- Skilled in crisis management, discharge planning, case management, and client communication within a healthcare setting
- Familiarity with EMR systems and CPT coding; experience working with individuals from diverse cultural backgrounds is highly valued
- Demonstrated ability to work effectively within interdisciplinary teams while adhering to social work practice standards and confidentiality protocols such as HIPAA
- Excellent interpersonal skills with the capacity for compassionate counseling across various age groups—including pediatrics when applicable—and developmental disabilities such as autism or dementia care
