Primary Care Physician – Geriatrics – Lake Mary, FL

Provenir Healthcare is working with an established healthcare organization in Lake Mary, Florida, seeking a Board-Certified or Board-Eligible Internal Medicine or Family Medicine Physician to join its outpatient team.
This is a great opportunity for a physician who enjoys building long-term patient relationships and practicing in a collaborative, value-based environment. The practice is dedicated to caring for seniors while giving physicians the opportunity to focus on preventive care, chronic disease management, wellness, and quality outcomes.
Position Highlights
- 100% Outpatient Primary Care
- Monday–Friday, 8:00 AM–5:00 PM
- Average 15 patients per day
- Primarily Senior/Medicare Advantage population
- Value-based care model focused on quality over volume
- Collaborative, team-oriented environment
- eClinicalWorks (eCW) EMR
- Geriatric Fellowship graduates welcome
Responsibilities
- Provide comprehensive primary care to adult and senior patients
- Evaluate and manage acute and chronic medical conditions
- Provide preventive care and wellness services
- Develop individualized treatment plans and manage medications
- Build lasting relationships with patients through continuity of care
- Collaborate with a team focused on value-based care and improved outcomes
Qualifications
- MD or DO
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Active, unrestricted Florida medical license
- Value-based care experience preferred
- Geriatric Fellowship graduates welcome
