About the Role
Heart to Heart Home Care is seeking a highly motivated, tech-savvy Intake & Eligibility Specialist to join our growing team. This role is ideal for someone who enjoys working with technology, solving problems, and helping patients navigate healthcare services.
The ideal candidate is detail-oriented, thrives in a fast-paced environment, learns new software quickly, and is comfortable managing multiple tasks while delivering exceptional customer service.
What You'll Do
- Manage new patient referrals from start to finish.
- Verify insurance eligibility and Medicaid coverage.
- Coordinate with hospitals, physicians, insurance plans, and community partners.
- Utilize multiple software systems to document patient information accurately.
- Track referral progress and proactively follow up to ensure timely admissions.
- Educate patients and families on available home care services.
- Schedule assessments and coordinate with clinical staff.
- Maintain accurate documentation while meeting productivity goals.
- Communicate professionally with internal teams and external partners.

- Healthcare, insurance, call center, intake, or customer service experience preferred.
Preferred Qualifications
- Experience with Medicaid, Medicare, MLTC, MAP, or home care services.
- Knowledge of EMR/EHR systems.
- Bilingual (English/Spanish) is a plus.
