We're No Different Home Health Agency is seeking experienced Home Health Registered Nurses (RNs) to complete Start of Care (SOC) assessments for skilled home health patients throughout our service area.

In this role, you will serve as the first point of contact for new admissions by completing comprehensive nursing assessments and helping establish individualized plans of care that promote each patient's health, safety, independence, and overall well-being.
This position provides assessments for Medicaid, managed care, commercial insurance, and private pay clients only. No Medicare, OASIS, or electronic charting is required. All documentation is completed using agency paper assessment forms.
COMPENSATION
- $65–$85 per completed Start of Care assessment

RESPONSIBILITIES
- Complete comprehensive Start of Care (SOC) nursing assessments for new home health patients.
- Evaluate each patient's medical history, current health status, functional abilities, home environment, and skilled nursing needs.
- Develop individualized plans of care based on physician orders and assessment findings.
- Obtain and document vital signs and perform comprehensive physical assessments.
- Identify patient and caregiver education needs and provide appropriate teaching regarding health, safety, medications, and disease management as applicable.
- Complete all required agency documentation accurately and within required timeframes using agency paper assessment forms.
- Communicate assessment findings, recommendations, and any immediate concerns with the Director of Nursing.
- Maintain patient confidentiality and comply with HIPAA, AHCA regulations, agency policies, and professional nursing standards.
- Provide compassionate, patient-centered care while promoting dignity, independence, and quality of life.
QUALIFICATIONS
- Current, unrestricted Florida Registered Nurse (RN) license.
- Home Health experience is required.
- Previous Start of Care (SOC) assessment experience preferred.
- Excellent nursing assessment, clinical judgment, and critical thinking skills.
- Strong verbal and written communication skills.
- Ability to work independently and manage time effectively.
- Professional, dependable, and compassionate.
- Reliable transportation, valid Florida driver's license, and current automobile insurance.
- Must successfully pass all required background screenings.
- Must be available to start immediately.
- Must have weekday flexibility to accommodate new patient admissions.
