Job Summary:

Responsibilities:
Provide direct patient care to residents in our skilled nursing facility, including administration of medications, wound care, and other treatments as needed
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct routine vital sign checks and document patient progress
Work closely with our nursing team to ensure effective communication and coordination of care
Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
Maintain accurate and detailed records of patient care and treatment

Qualifications:
Current LPN license in the state of Michigan
High school diploma or equivalent required
1+ year of experience in a long-term care setting preferred
Ability to provide excellent patient care and communicate effectively with patients and families
Strong critical thinking and problem-solving skills
Ability to work effectively as part of a team and adapt to changing circumstances
