Qualifications:
- Current Ohio State PRN License (Required)
- Minimum of 1 year of nursing experience, preferably in homecare
- Current CPR certification (Required)
- BCI Fingerprinting (Required)
- Negative TB test within the last 12 months (Required)
- Experience with State Medicare waiver programs
- Exceptional communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently
Duties and Responsibilities:
- Assess patients' health status and record pertinent information
- Develop and update personalized home care plans
- Provide basic patient care and assistance, ensuring patient comfort and safety
- Educate patients and their families about home healthcare routines and strategies
- Coordinate with healthcare team members to relay patient status
- Adhere to professional nursing ethical standards and respect patient confidentiality at all times
