Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, ensuring compliance with healthcare regulations, and providing support for patients with complex medical needs. This role requires a strong understanding of medical terminology, patient care, and the ability to work collaboratively within a multidisciplinary team.
Responsibilities
- Conduct comprehensive assessments of patients' health status and needs.
- Develop individualized care plans in collaboration with patients, families, and healthcare providers.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Provide education to patients regarding their conditions, treatment options, and self-management strategies.
- Ensure adherence to HIPAA regulations and maintain patient confidentiality at all times.
- Coordinate services across various healthcare settings including hospitals, outpatient facilities, and home care.
- Advocate for patients by facilitating access to necessary resources and services.
- Document all interactions and updates in accordance with established protocols.
- Stay informed about current practices in infectious disease care and critical care management.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Strong knowledge of physiology and medical terminology relevant to patient care.
- Experience in managed care or case management preferred.
- Familiarity with infectious disease care is a plus.
- Excellent communication skills with a focus on patient service.
- Ability to work collaboratively within a team environment while managing multiple priorities.
- Critical thinking skills to assess complex situations and make informed decisions.

Job Type: Full-time
Pay: $73,000.00 - $79,000.00 per year
