What You’ll Do:
- Perform Start of Care visits and in-home OT treatments
- Create and manage individualized care plans
- Collaborate with an interdisciplinary team
- Document using HomeCare HomeBase (HCHB)
Requirements:
- Colorado OT license
- Home health experience preferred
- Strong communication and documentation skills
- Driver’s license, insurance, CPR certification
