Manage and oversee a designated home health patient caseload from an office setting

Coordinate interdisciplinary patient care with registered nurses, therapists, social workers, and physicians

Review and approve clinical documentation for accuracy, completeness, and regulatory compliance

Communicate with the field clinicians/DON to decide the initial plan of care based on initial patient assessment and physician’s orders

Monitor patient outcomes and identify opportunities to improve quality of care

Participate in clinical outcomes monitoring, follow up and agency performance improvement initiatives

Provide clinical guidance and support to field clinicians

Address clinical concerns, patient complaints, and care coordination issues in a timely manner

Gain quality experience in working with a multidisciplinary team and personal patient interaction

Participate in quality assurance and performance improvement (QAPI) activities
