HeartShare St. Vincent’s Services is currently seeking a Family Team Conference (FTC) Facilitator for our Foster Care program. The Family Team Conference (FTC) Facilitator is responsible for guiding and tracking all conferences as required by the Administration for Children’s Services (ACS) utilizing the Family Team Conference Model. The facilitators organize and lead various types of Family Team Conferences, such as service plan reviews, foster placement stabilization meetings, and discussions about permanency planning goals. Utilizing strong engagement skills, the facilitator is expected to yield positive safety and permanency outcomes for children and families with the input from a multidisciplinary team and various stakeholders. This position also supports engagement with families by facilitating conferences, assisting with Power of Two referrals and referrals that strengthens permanency outcomes.
Actively guide the family team conference, ensuring that families, key stakeholders, community members, and service providers are involved and engaged throughout the conference.
Plan and ensure timely distribution of family team conference summaries to all participants.
Schedule and coordinate Family Team Conference with internal and external key stakeholders.
Conduct pre-conference meetings and post-conference debriefings to evaluate outcomes. · Perform follow-up reviews to monitor progress on tasks identified during conferences.
Assess family strengths and needs in collaboration with the interdisciplinary team and key stakeholders.
Identify, address safety, and risk concerns during conferences to aid in development of service plan goals.
Assist with & complete accurate, timely documentation in Connections and other agency databases.
Provide ongoing education to clients and employees about Family Team Conferencing to enhance understanding and effectiveness.
Generate weekly reports to track and distribute to the case planning team to ensure all conferences are completed on time.
Monitor outcomes related to identifying performance trends, best practices, and opportunities for improvement.
Conduct Quality Assurance activities related to Family Team Conferencing (i.e., monitor the quality of the FTC summaries and outcomes).
Assist with developing service plans using SMART Goals and recommendations based on the collective input from families and stakeholders, focusing on fostering permanency, safety, and overall wellbeing for children and families.
Prepare for each Family Team conference by performing a comprehensive review of case records and other relevant documentation.
Serve as the FTC liaison between provider agency and ACS (Administration for Children’s Services).
Participate in supervision, staff meetings, training, ACS Facilitated Family Tea Conference, and planning sessions as required.
Responsible for ensuring adherence to all agency policies, procedures and governmental regulations, including but not limited to HIPAA.
Performs other job-related duties as assigned, consistent with the scope, responsibilities, and level of the position, and in compliance with applicable laws, regulations, and organizational policies.
BA/BS/BSW Required; master's degree preferred.
Ability to work collaboratively with interdisciplinary teams.
One year knowledge of NYC Children’s Services Family Team Conferencing model or group facilitation experience preferred.
Strong clinical, organizational, written, verbal, and administrative skills with the ability to manage multiple assignments.
Strong engagement, leadership, interpersonal, communication, and conflict resolution skills.
Commitment to assessing, permanency, safety, well-being, and addressing risks in family dynamics.
Computer literate, Microsoft Office & Connections knowledge preferred
Bilingual in English and Spanish a plus.
