The Case Management team plays a vital role in helping residents navigate challenges, build self-sufficiency, strengthen community connections, and achieve long-term housing stability.
What Success Looks Like
This is a hands-on leadership role that combines employee supervision, program oversight, and direct support for complex client situations. Successful supervisors are collaborative, organized, and committed to delivering high-quality services through a Housing First and trauma-informed approach. They build strong teams, coach employees toward professional growth, and use data and quality assurance practices to strengthen program outcomes.
Success in this position means leading a team that consistently delivers responsive, client-centered services that promote housing retention and well-being. You'll support employees in navigating complex situations, strengthen partnerships with internal and external stakeholders, ensure compliance and documentation quality, and help create an environment where both clients and employees can thrive.
The Role

Supervise, coach, and develop Case Managers and Tenant Services Coordinators through regular supervision, training, mentorship, performance management, and professional growth opportunities.

Provide hands-on support for complex client situations, including case consultation, crisis response, joint meetings, problem-solving, and service coordination.

Facilitate Multidisciplinary Approach to Services (MAS) meetings and coordinate care planning with internal teams, property management, community providers, and external service partners.

Promote Housing First, trauma-informed, strengths-based, and client-centered practices across all aspects of service delivery.

Review and monitor caseloads, service plans, referrals, assessments, client documentation, and HMIS entries to ensure quality, timeliness, accuracy, and compliance.

Conduct file reviews, documentation audits, and quality assurance activities to support adherence to organizational, contractual, and funding requirements.

Monitor program performance, housing outcomes, and service data to identify trends, strengthen operations, and support continuous improvement efforts.

Carry a caseload and provide direct case management services as needed to support staffing, operational needs, housing stability, and positive client outcomes.

Foster a team culture of accountability, collaboration, continuous learning, and service excellence.
Work Schedule
This is a full-time leadership position supporting Permanent Supportive Housing programs located throughout the community. The typical schedule is Monday through Friday; however, operational needs may occasionally require schedule flexibility to support employee supervision, client needs, crisis response activities, meetings, or program operations across multiple locations.
Compensation & Benefits
The anticipated starting pay range for this role is $27.63 to $31.77 per hour, paid bi-weekly. Final placement and starting salary will be determined based on the candidate's experience, qualifications, certifications, and internal equity considerations.
