Join our dynamic healthcare team as a Home Health Physical Therapist, where you will play a vital role in delivering personalized rehabilitation services to patients in their homes and community settings. The Physical Therapist is responsible for providing skilled physical therapy services to patients in their place of residence in accordance with the physician-ordered plan of care, Medicare Conditions of Participation (42 CFR §484), and Indiana state practice regulations. The Physical Therapist evaluates patients, develops treatment plans, provides therapeutic interventions, and coordinates care to improve functional mobility, safety, and independence. As a key member of our interdisciplinary team, you will utilize your expertise in physiology, anatomy, and medical terminology to facilitate effective treatment and discharge planning. Your dedication will directly impact patient outcomes by providing compassionate, high-quality care.
Responsibilities
- Perform initial and ongoing physical therapy assessments
- Evaluate functional mobility, strength, balance, gait, safety, and rehabilitation potential
- Complete OASIS assessments as assigned and within required timeframes
- Identify patient goals and rehabilitation needs
- Develop individualized physical therapy plan of care in collaboration with physician and interdisciplinary team
- Establish measurable treatment goals
- Recommend frequency and duration of therapy services
- Update plan of care based on patient progress
- Provide skilled therapy services including but not limited to: Therapeutic exercise Gait training Balance and coordination training Transfer training Neuromuscular re-education Pain management techniques Fall prevention interventions Assistive device training Home safety evaluation Home Health Physical Therapist Job Description Functional mobility training
- Educate patient and caregiver on: Disease process Home exercise program Fall prevention Safe mobility techniques Equipment use
- Complete all documentation timely and accurately per agency policy and Medicare regulations
- Document skilled need, progress, and medical necessity
- Complete OASIS assessments when required
- Document communication with physician and care team
- Complete discharge summaries
- Communicate patient status to physician and interdisciplinary team
- Participate in care conferences
- Coordinate with nurses, occupational therapists, speech therapists, and home health aides
- Report changes in patient condition promptly
Experience
Required
- Graduate of an accredited Physical Therapy program approved by: Commission on Accreditation in Physical Therapy Education
- Current Physical Therapist license in the State of Indiana
- Current CPR certification
- Valid driver’s license
- Reliable transportation
- Auto insurance
- Ability to travel to patient homes
- Ability to meet physical demands of home health
Preferred
- Minimum one (1) year clinical experience
- Home health experience preferred
- Knowledge of Medicare home health regulations and OASIS

Pay: $40.00 - $50.00 per hour
