Aging Waiver Case Manager
Medicaid Aging Waiver program is an in-home case management program. The case manager will arrange services for qualified seniors age 65 or older and on Medicaid to avoid permature institutionalization. These arranged services may include personal care, homemaking, adult day care, respite care, chore serivices, meals, adult companion, non-medical transportation, or medical equipment. The case manager will be required to visit each client each month, arrenge services, keep up with documentation, and bill Medicaid. This position requires a SSW or related license.
State Health Insurance Information and Assistance Progam(SHIP)/Senior Medicare Patrol (SMP)
Help Individuals navigate the Medicare system through one-on-one counseling and outreach events including submitting Medicaid applications. Also, educate seniors about health care fraud and abuse.
Extensive training will be provided for both programs.
A bachelor's degree in social work or related field, SSW or greater license, valid Utah driver's license, and basic computer skills are required. A plus would be at least one year working with elderly population or younger people with disabilities and can speak Spanish.
