Caring Hands Adult Medical Day Care is seeking an energetic, compassionate, and results-driven Community Outreach Specialist to help connect older adults and individuals with disabilities to our comprehensive Adult Medical Day Care services.

Responsibilities
- Develop relationships with physicians, hospitals, rehabilitation centers, skilled nursing facilities, home health agencies, assisted living communities, social workers, case managers, and community organizations.
- Educate healthcare professionals and community partners about our Adult Medical Day Care services.
- Conduct presentations, health fairs, educational events, and community outreach activities.
- Generate qualified referrals and maintain a consistent referral pipeline.
- Build and maintain strong relationships with referral sources.
- Track outreach activities and referral outcomes using CRM or other reporting tools.
- Collaborate with the admissions and clinical teams to ensure a smooth enrollment process.
- Represent Caring Hands professionally throughout the Baltimore community.
Qualifications
- Experience in healthcare marketing, community outreach, business development, or sales preferred.
- Experience working with Adult Medical Day Care, home care, hospice, rehabilitation, hospitals, managed care, or senior services is highly desirable.
- Excellent communication and interpersonal skills.
- Self-motivated, organized, and goal-oriented.
- Ability to work independently while contributing to a team environment.
- Valid driver's license and reliable transportation required.
- Bilingual (English/Spanish) is a plus but not required.

- Competitive salary: $50,000–$60,000 annually (based on experience)
