Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
- Utilize electronic health record (EHR) systems such as Epic, Point Click Care , or Athenahealth to document patient information meticulously and ensure compliance with HIPAA regulations.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques.
- Provide specialized care for patients with conditions like Alzheimer's disease, dementia, infectious diseases, or those requiring hospice or palliative care.
- Support emergency situations by managing airway management, ventilator use, acute pain management, and triage protocols in fast-paced environments like Level I Trauma Centers or emergency rooms.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care coordination to optimize patient recovery and safety.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in good standing with the state licensing board.
- Demonstrated experience with EMR/EHR systems such as Epic, PCC, or Athenahealth for accurate medical documentation.
- Critical care experience preferred, including ICU or Level I Trauma Center settings; hospital medicine or emergency medicine background is highly valued.
- Proficiency in performing medical procedures such as phlebotomy, venipuncture, catheter care, and sterile techniques.
- Knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding for billing purposes and diagnostic evaluation.
- Experience working with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities or behavioral health needs is a plus.
- Strong communication skills for patient assessment and health coaching; ability to work effectively within multidisciplinary teams.
