Job Summary
Join our dynamic healthcare team as a Part-Time Cardiology Nurse Practitioner specializing in home visits! In this energetic role, you will deliver expert cardiovascular care directly to patients in their homes, ensuring comprehensive assessments, treatment plans, and ongoing management. This is a part time position with very flexible hours, with increase in patient load as the practice develops and as desired. The opportunity allows for setting days of the week and times of the day that work best for the provider. Would anticipate developing into 2-3 days per week with up to 8-12 patients per day.
Duties
- Conduct thorough cardiovascular assessments during home visits, including vital signs, physical examinations, and patient history reviews.
- Develop and implement individualized treatment plans in collaboration with physicians and other healthcare professionals.
- Administer medications, including injections and IV infusions, following all safety protocols and regulations.
- Utilize electronic health record (EHR) eClinicalWorks to document patient encounters accurately and efficiently.
- Educate patients and their families on heart health management, medication adherence, lifestyle modifications, and symptom monitoring.
- Perform specialized procedures such as suturing or catheter care when necessary during home visits.
- Coordinate with interdisciplinary teams for case management, discharge planning, and follow-up care to ensure continuity of treatment.
Requirements
- Valid Nurse Practitioner license with certification in cardiology or related specialty.
- Prefer, but not required, 1-2 years experience as cardiology NP.
- Strong knowledge of cardiac diagnostics including echocardiography, sonography, and cardiac catheterization procedures.
- Familiarity with EMR/EHR systems such as Cerner or Athenahealth for medical documentation.
- Experience with telehealth platforms to support remote patient monitoring and consultations.
- Excellent patient assessment skills combined with strong communication abilities for effective patient education.
- Knowledge of HIPAA regulations to ensure patient privacy and confidentiality at all times.
