Are you a bilingual (English/Spanish) Nurse Practitioner or Physician Assistant looking for a predictable weekday schedule and the opportunity to build meaningful relationships with older adults? We're hiring a Bilingual Nurse Practitioner (NP) or Physician Assistant (PA) in Springfield, MA, to provide primary care services through a Program of All-Inclusive Care for the Elderly (PACE).
This role is ideal for an NP or PA who enjoys caring for medically complex geriatric patients and wants the time and support to deliver thoughtful, patient-centered care.

What You'll Do
- Provide primary and preventive care to PACE participants in clinic, home, and community settings
- Conduct comprehensive assessments and develop individualized treatment plans
- Manage chronic conditions and coordinate ongoing care
- Order, interpret, and follow up on diagnostic tests and lab results
- Collaborate with physicians, nurses, therapists, social workers, pharmacists, and other care team members
- Educate participants and caregivers on medications, disease management, and preventive care
What You'll Bring
- Current Massachusetts Nurse Practitioner or Physician Assistant license
- National certification as an NP or PA
- DEA registration or ability to obtain
- Fluency in English and Spanish, both spoken and written
- At least one year of clinical experience as an NP or PA
- Strong communication and patient education skills
- Valid driver's license and reliable transportation for occasional home visits (approximately five per month)
