Join a mission-driven Assertive Community Treatment (ACT) team dedicated to helping individuals with serious mental illness achieve wellness, stability, and independence. This role is built for a nurse who wants to be out in the community, building real relationships, and making a tangible difference, not stuck behind a desk.
What You'll Do
- Deliver hands-on health assessments, medication management, and wellness support directly in clients' communities
- Administer oral and intramuscular medications and monitor for side effects and adherence
- Perform phlebotomy and coordinate broader healthcare services for clients
- Provide psychoeducation to help clients understand their medications and overall health
- Conduct mental status assessments and recovery-oriented check-ins
- Rotate participation in the team's after-hours emergency support line
- Partner closely with the team's prescriber on medication planning and healthcare coordination
- Build collaborative relationships with families, housing providers, hospitals, shelters, criminal justice partners, and Assisted Outpatient Treatment (AOT) contacts
- Bring a trauma-informed, culturally responsive, and motivational interviewing approach to every interaction
- Participate actively in team and clinical meetings to support whole-person care planning
- Maintain accurate, timely documentation (progress notes, assessments, incident reports) in line with state and city mental health and Medicaid regulations
- Take on additional responsibilities as directed by team leadership
What We're Looking For
- Active Registered Nurse (RN) license in New York State
- Minimum 3 years of experience in psychiatric nursing, substance use treatment, or social service settings supporting individuals facing homelessness, justice involvement, or limited social support
- Strong knowledge of community-based treatment models, support services, and local resources
