Community Support Worker (CSW) – Comprehensive Community Support Services (CCSS)
Journey to Wellness is seeking compassionate and motivated Community Support Workers (CSWs) to join our Comprehensive Community Support Services (CCSS) program.
Are you passionate about helping individuals and families overcome barriers, build life skills, and connect with their communities? Our CCSS team provides meaningful, community-based support to children, adolescents, adults, and families experiencing behavioral health challenges.
As a Community Support Worker, you will work alongside clients to identify strengths, develop goals, and provide practical support that promotes recovery, resiliency, independence, and improved quality of life.
Position Responsibilities:
- Provide community-based support services aligned with each client’s individualized CCSS service plan.
- Assist clients in developing skills related to:
- Independent Living
- Learning/Education
- Employment and Vocational Goals
- Social Skills and Community Connections
- Recreation and Healthy Lifestyle Activities
- Help clients identify and overcome barriers impacting daily functioning.
- Connect clients and families with community resources and supportive services.
- Provide skill-building interventions, coaching, advocacy, and care coordination.
- Assist clients with developing crisis prevention, coping strategies, and safety plans.
- Provide crisis intervention support, including de-escalation and linkage to appropriate emergency or behavioral health resources when needed.
- Collaborate with clinical counselors, families, schools, community providers, and other support systems to support client goals and continuity of care.
- Complete timely, accurate, and thorough documentation of services provided in accordance with program and agency requirements.
Qualifications:
Applicants must meet New Mexico CCSS Community Support Worker requirements, including one of the following:
- Bachelor’s degree in a human services-related field with relevant experience; OR
- Associate degree in a human services-related field with relevant experience; OR
- High school diploma/GED with 4 years experience working with the target population
Additional requirements include:
- Must be 18 years of age or older.
- Ability to complete required documentation accurately and on time.
- Willingness to collaborate closely with clinical counselors and other members of the treatment team.
New Mexico CCSS requirements also include completion of required CCSS training within the designated timeframe after hire.
Preferred Qualifications:
- Experience working with children, adolescents, adults, and/or families impacted by behavioral health challenges.
- Experience with case management, community resources, behavioral health, education, foster care, CYFD, or related systems.
- Strong communication, organization, and documentation skills.
- Ability to work independently in community settings.
- Bilingual (English/Spanish) preferred but not required.

Pay: $18.00 - $30.00 per hour
