Job Description (Other duties may be assigned)
· Assists the patient in the activities of daily living (ADL), including personal care, hygiene, baths, shampoo, planning/preparing meals, eating, dressing, ambulation and changing/making patient’s bed
· Encourages the family members to participate in patients’ care while giving and providing support
· Provides companionship to the patient
· Assists in the maintenance of a safe and healthy environment while using equipment and supplies safely and properly
· Reminds patient of medication as ordered by the physician
· Takes and records accurate patient vital signs when advised
· Informs Registered Nurse (RN) and/or RN designee of changes in patients’ condition
· Document activities and findings while submitting documentation as required by company procedures
Required documents (We cannot accept expired or illegible documents)
· CNA License/ HHA Certificate
· Health Statement (within 6 months that states you are "free of communicable diseases." Usually a TB test or chest X-ray.)
· Driver’s License or Identification Card
· Social Security Card
· CPR Card (American Heart Association, American Safety & Health Institute, or Red Cross)
· Level 2 Background
· HIV/Aids Certificate (2 hours)
· Assisting with Self-Medication CEU (2 hours)
· Alzheimer's CEU (3 hours)
· Auto Insurance Policy (if willing to transport clients)
