Position Overview
You’ll provide skilled nursing care in the home setting following physician-ordered care plans. Responsibilities include patient assessments, medication administration, documentation, care coordination, and patient education to support improved health outcomes.
Key Responsibilities
- Perform skilled nursing visits and treatments within RN scope of practice
- Complete OASIS documentation accurately and on time
- Monitor, document, and report changes in patient condition
- Coordinate care with physicians and interdisciplinary teams
- Supervise LPNs and HHAs as assigned
- Educate patients and families on disease management and medications
Qualifications
- Active Ohio RN license
- CPR certification (BLS or ALS)
- Home health experience & OASIS knowledge strongly preferred
- Minimum 1 year home care experience or 1 year recent hospital/SNF experience
- Strong assessment, documentation, and communication skills
- Valid driver’s license, auto insurance, and reliable transportation
- Meets all state and federal employment requirements
- Current TB test or chest X-ray upon hire
