The Washington State Department of Corrections is seeking a highly motivated and qualified individual for the Corrections Specialist 2 – Reentry Care Coordinator position at the Monroe Correctional Complex located in Monroe, WA.
The goal of the Reentry Care Coordinator is to provide support with healthcare transition planning as part of the Health Services Reentry Team. This position is assigned to the Monroe Correctional Complex and reports to the Psychiatric Social Worker 4. This position is performing journey-level work coordinating reentry care and requires strong interpersonal skills to make connections and build relationships with incarcerated individuals and DOC staff. The Reentry Care Coordinator works collaboratively with the social worker, nursing staff, the primary care provider, patient services representatives and other team members with the purpose of making the critical link to health information/services and assists with care coordination. The Reentry Care Coordinator is flexible and willing to work in a dynamic and evolving role according to individual patient and program demands, has strong verbal/written communication skills, and strong organizational skills. To work with the patient and their Health Services Reentry Team to eliminate barriers to continuity of care from prison to community.

Develop, coordinate and implement healthcare reentry planning for patients releasing from prison:

Track releasing patients and their transition of care needs using the statewide reentry tracker and triaging health services reentry kites.

Organizing multidisciplinary team meetings.
-
Attend reentry team meetings, wrap around meetings, and other meetings related to transitioning individuals from prison to community.

Coordinates and facilitates resources to support transition of care to community.
-
Provide Narcan education and other education.
-
Partner with Health Services team members to develop processes to ensure individuals.
-
Communicate with Managed Care Organizations.
-
Explain and clarify any applicable laws, regulations, policies, and procedures.
-
Provide liaison services. Assist clients in completing forms and applications (to include prison and community programs, benefits and resources).

Partner closely with clinic team/providers/therapists.
-
Gather documentation needed from health record for Health Services Reentry Team when requested to facilitate access to care, programs, and benefits.

Update SharePoint sites and evaluate program.

Partner, plan and coordinate with Community Corrections, Prisons, and Reentry Divisions to ensure communication and continuity of care.

Prepare release packets in coordination with Health Services Reentry Team.
Reduce access to care barriers and improve care coordination for patients releasing from prison:

Establishes and maintains community partnerships including internal and external stakeholders.

Eliminate barriers associated with care coordination.
-
Engages physicians, hospital staff, leaders and community partners in analysis, intervention and resolution activities.

Works with colleagues to eliminate avoidable delays.
-
Refer quality, infection control and risk management issues to appropriate individual or department.

Staff’s complex cases with Health Services Reentry team to facilitate.

Builds relationships with an understanding of community resources and support. Evaluate programs to include reviewing and analyzing data, identification of trends, and making recommendations to team, supervisor, and management.
Responsible for serving as patient liaison and advocating for patients to achieve quality patient focused outcomes:

Assists patients along their care pathway by serving as an advocates.

Shares assessments and patient response; remains a support system throughout the patient’s course of treatment.

Provides education and logistical support.

Collaborates with the patient, patient representatives and interdisciplinary team.
Actively Participate in Clinical/Facility/Statewide Coordinated Quality Improvement Meetings, Projects and Programs:

Maintains current knowledge of departmental policies/standards/procedures, new clinical procedures/processes, patient safety goals, core measure quality indicators and other issues/requirements.

Participate in required trainings and attend department team meetings.

Develop and maintain processes.

Required Qualifications:

Two (2) years of experience in a health care setting.
-
Three (3) years of professional experience in adult corrections, social services or a closely related field.

Experience with case management for physical and/or behavioral health needs.

Proficiency using Microsoft Office Software (Word, Excel, Outlook).

Excellent written and verbal communication skills.
-
Knowledge of community resources.
Desired Qualifications:
-
Satisfactory completion of two years college or Bachelor’s Degree in social services related field from an accredited college or university whose accreditation is recognized by the US Department of Education or Council for Higher Education (CHEA).
